The Aperion Caregivers consistently follow, evaluate, and modify this plan to ensure that your personal, medical, and rehabilitation needs are taken care of while you recover and prepare to return home. Post-hospital and rehabilitation guests at Aperion Care Forest Park receive 24-hour skilled nursing care, as well as therapy available 7 days per week. Our spacious, modern facility features an innovative Congestive Heart Failure (CHF) program, Bridge to Health, with a cardiologist doing weekly rounds in-house.
