About Us

  • The Hospice Team consists of a nurse, physician, medical social worker, chaplain and others involved in the care of each individual. The team's role is to monitor all phases of care, treatment, medication, and services provided. The team makes recommendations to continue, alter, or change techniques in order to assure patient comfort and pain control. In all circumstances, each patient receiving hospice services has complete control of the care they receive.

    In hospice, we provide assistance to the patient and to the family members. We proudly adhere to the Hospice Bill of Rights and Responsibilities. Families who come to Horizon Hospice for help receive services customized to their needs. Additionally, we are happy to make available two related documents which are covered during our first visit: the Consent Form and Election Form.
