• The Monadnock Adult Care Center is located at 22 North Street in Jaffrey, New Hampshire. Our mission is to improve the quality of life of participants, families, and caregivers while supporting an individual’s efforts to remain independent. The Monadnock Adult Care Center offers structured care during the day to adults with physical, emotional, or social challenges.
