Our goal is to work in interdisciplinary teams to care for all the needs of our patients and residents: physically, emotionally, nutritionally, spiritually. Our core services include: Transitional Care Services, Long-Term & Memory Care & Outpatient Therapy.
  Rehabilitation:
  Our rehabilitation program is tailored to each individual. Therapists are available 6 days a week to provide one-on-one sessions.
  Physical Therapy:
  Improves mobility and ambulation by addressing pain, balance, strength, flexibility and coordination.
  Occupational Therapy:
  Promotes the highest level of independence by addressing safety, cognition, endurance and upper body strength/range of motion. Daily activities include bathing, dressing and meal preparation.
  Speech Therapy:
  Specializes in disorders that involve speech, language, voice, cognition and swallowing.
  Nursing:
  A dedicated team of registered nurses, licensed practical nurses, and nursing assistants coordinate your care and services with other members of the interdisciplinary team. Licensed nurses provide oversight, medication management, pain management, and wound management. The provision of all nursing cares and services provided is in accordance with the individualized plan of care.
