Transitions Healthcare’s founders wanted to create a system that was more responsive to patients, with stronger continuation for physicians’ treatment plans and closer coordination with hospitals. Our four principals come from the fields of Clinical Nursing, Finance and Nursing Home Administration. Their combined focus is on dedicating earnings to maximize care.
