• The L.P. Johnson Family Health Center (FHC) is a comprehensive family medicine teaching facility associated with the University of Illinois College of Medicine at Rockford Family Medicine Residency Program.The FHC is staffed with board-certified faculty physicians who supervise resident physicians. The presence of licensed nurse practitioners with advanced academic and clinical experience enhances delivery of quality care and education. Our staff speaks multiple languages. LP Johnson Family Health Center has earned the highest national recognition for patient-centered care. FHCs Patient-Centered Medical Home, Your Care, is a model of primary care that combines teamwork and information technology to improve care, improve patients experience of care and reduce costs.
